- Current Coverage Details: If you’re renewing, please bring your Marketplace login or any plan information from 2025.
- Doctor & Prescription List: Your preferred physicians, clinics, and any regular prescriptions so we can match network and formulary coverage.
- Immigration or Citizenship Documents: If applicable, please have a Green Card, work permit, or naturalization document available.
